This seems to overlook the end user who takes mail by SMTP, not IMAP
or POP.  Or is there a suitable alternative to a Mail Transport Agent
(MTA) (such as exim) for such a user?

There are clear advantages of SMTP over IMAP or POP for mail receipt:

   o Unlimited supply of distinct email addresses per user.
   o No ISP meddling with incoming mail.
   o Instant delivery.

I'm surprised that more are not demanding it.  I promote it when I can
because it increases the constituency for static IP addresses.

The case for using IMAP or POP instead of SMTP for mail receipt is
weakened in this age of always-on Internet connections for end users
and the availability of static IP addresses.
